• Fill out the claim form with all necessary details and submit it along with the original prize-winning ticket/s within 30 days from the draw date or result announcement.

1. Required Documents:

• Enclose an affidavit, PAN card, one ID proof, 4 passport-sized photos, and a canceled cheque leaf of your bank account with the claim form[5].
• Ensure the form includes your name, address, lottery details, prize amount, PAN number, contact number, and bank account information like account number and IFSC code.

1. Instructions for Claiming Prize Money:

• Submit separate claim forms for each ticket along with all necessary documents as mentioned.
• Provide the original prize-winning ticket with a photocopy attested by a Gazetted Officer/Notary Public, four passport-sized photos, and an affidavit of ownership of the ticket from a 1st Class Magistrate/Notary Public.

1. Prize Disbursement:

• The Director will disburse prize money for accepted genuine claims after deducting necessary amounts like income tax.
